At RMHW, we offer flexible scheduling which often allows us to make same day appointments. In order to provide all our patients with the care they deserve, please notify us 24 hours in advance if you are unable to keep your scheduled appointment. We accept and bill most private insurance plans.
There has been a recent change in policy here at Rocky Mountain Health & Wellness, effective immediately. Due to contractual obligations between the patient and their insurance company, it is required from us to collect your co-payment amount at the time of your visit.
If you are a self-pay patient, you will be required to pay the full amount due for your visit at the time of check-out.
If you are unable to supply your co-pay or self-pay amount at the time of your scheduled visit, you are now required to reschedule your appointment to a later time or date. Rocky Mountain Health & Wellness is unable to accept any postdated payments/checks.

Here are some things you can do before your first appointment to ensure you’re seen quickly and efficiently.
STEP 1: COMPLETE AND SUBMIT NEW PATIENT FORMS
You may access your personal, protected health information in our new Patient Portal.
Current patients, please contact our office for your username and password.
Please continue to contact your pharmacy FIRST for prescription refills.

Please print and complete the following forms prior to your first appointment. If some seem a little long, that’s because we want to get a complete, in-depth look at your health even before your first visit.
STEP 2: RETURN YOUR COMPLETED NEW PATIENT FORMS
If possible, at least one week prior to your appointment, return the completed forms to our offices. That will ensure you are seen as quickly as possible after you’ve walked through our front doors.
