We’re thrilled to meet you, and are looking forward to working with you to permanently improve the health and wellness of you and your family.

At RMHW, we offer flexible scheduling which often allows us to make same day appointments. In order to provide all our patients with the care they deserve, please notify us 24 hours in advance if you are unable to keep your scheduled appointment. We accept and bill most private insurance plans.
There has been a recent change in policy here at Rocky Mountain Health & Wellness, effective immediately. Due to contractual obligations between the patient and their insurance company, it is required from us to collect your co-payment amount at the time of your visit.

If you are a self-pay patient, you will be required to pay the full amount due for your visit at the time of check-out.
If you are unable to supply your co-pay or self-pay amount at the time of your scheduled visit, you are now required to reschedule your appointment to a later time or date. Rocky Mountain Health & Wellness is unable to accept any postdated payments/checks.

Here are some things you can do before your first appointment to ensure you’re seen quickly and efficiently.

 

STEP 1: COMPLETE AND SUBMIT NEW PATIENT FORMS

Option 1:
You may access your personal, protected health information in our Patient Portal.
Current patients, please contact our office for your username and password.
Please continue to contact your pharmacy FIRST for prescription refills.
Option 2:
Please print and complete the following forms prior to your first appointment. If some seem a little long, that’s because we want to get a complete, in-depth look at your health even before your first visit.

STEP 2: RETURN YOUR COMPLETED NEW PATIENT FORMS

If possible, at least one week prior to your appointment, return the completed forms to our offices. That will ensure you are seen as quickly as possible after you’ve walked through our front doors.

Here are some things you can do before your child’s first appointment to ensure you’re seen quickly and efficiently.

 

STEP 1: COMPLETE AND SUBMIT NEW PATIENT FORMS

If your child is 11 years or younger, please print and complete the following forms prior to your first child’s appointment. If your child is 12 years of age or older, please complete our adult paper work. If some seem a little long, that’s because we want to get a complete, in-depth look at your child’s health even before their first visit. Please either bring a copy of your child’s previous medical record, including vaccination record, or complete the Record Release form below.

Are You Sick Today and Want To Be Seen?
Call and make a same-day appointment!

One of our providers is always available to see you in our clinic if you are acutely ill. If you make a same-day appointment, please logon to your patient portal and complete the questionnaires section of the website or print out the new patient paperwork and bring it with you to your appointment.

Acute Illness that can be seen the same day:

  • Sore Throat
  • Cough/Cold
  • Pink Eye
  • Sinus Infection
  • Urinary Tract Infections
  • Rashes
  • Ear Aches
  • Sprain/Strain
  • Abdominal pain
  • Nausea/Vomiting

 

Please note that we are not an urgent care facility. If you have one of the following conditions, please go to the nearest emergency department or urgent care.

  • Chest pain
  • Severe shortness of breath
  • Injury potentially requiring sutures
  • Head Injury
  • Motor Vehicle Accident (MVA)
  • Concussion
  • Uncontrolled bleeding
  • Sudden or severe pain
  • Sudden weakness, dizziness or changes in vision
  • Coughing or vomiting blood
  • Severe or persistent vomiting or diarrhea
  • Change in mental status, such as confusion or disorientation
